Dear Ms. Sophie Chloe,

Re: Mr. Robert Griffith, 64-years- old

Mr. Robert Griffith is being discharged today, back into your care after undergoing treatment for a bout

of pneumonia and bronchitis.

You will be delighted to know that, Mr. Robert’s condition has significantly improved. His vitals have been

normalized and pneumonia is under well control. Despite having an occasional cough, he is able to

breathe normally.

On admission, he was found to have tachycardia and tachypnea. In addition, he had adventitious lung

sounds on auscultation. He was commenced on Injection Amoxicillin and Deriphyllin which need to be

continued for 5 days.

Upon discharge, he will require nebulization with Asthalin and adequate rest. He is advised to

increase oral fluid intake. It would be greatly appreciated if you could encourage deep

breathing exercises.
